The New Patient Process
Get a Doctor's Referral
Contact your pediatrician to obtain a referral for therapy services. Without a doctor's referral, most insurance companies will not cover the cost of services.
We'll Contact You
Within 2 business days of receiving your referral, someone from our office will contact you to confirm receipt and discuss scheduling an evaluation. We'll let you know about available openings or waitlist status.
Schedule Your Evaluation
We will call you to schedule the evaluation appointment at a time that works for your family. Please allow approximately one to two hours for the evaluation.
Complete Your Forms
Please complete and return all required forms at least 2 days before your scheduled evaluation. Refer to the Forms by Therapy Type section below to complete the respective forms.
To ensure continuity of care and provide the most appropriate treatment, please complete the Release of Information form prior to establishing care, if your child has received a therapy type at another clinic within the past year.
